A copy of the member’s social security card and driver’s license (or other government issued ID) must be submitted with the completed MERS form titled “Refund Application Packet.” The processing of the member’s refund will be delayed if the identification documents are not received with the application.
For a direct refund to a member, one of the following must be attached:
a. A voided check (pre-printed) for a checking account in the member’s name
b. Bank issued direct deposit form.
